The condition may be most devastating to teens, but it can strike at any age. In fact, nearly 50 percent of all men will experience gynecomastia at some point in their lives, Many sufferers don’t know that gynecomastia is a medical condition caused by hormone fluctuations and weight gain. Nor do they know that exercise or weight loss alone may not be effective in getting rid of male breasts once they develop.

One of the biggest culprits for adults—and one that’s easily avoidable—is weight gain. When men gain weight, they’re not just getting bigger; the extra weight is actually changing their hormone production—and not in a way that most men would like. By nature, fat tissue manufactures the female hormone estrogen, which in turn stimulates breast tissue—meaning that significant weight gain will always be associated with some sort of breast growth, even in men, according to experts. A person who’s overweight will be fatter all around, and maybe even develop the excessive breast fat that doctors call pseudogynecomastia—the appearance of male breasts caused by fat alone. Most really overweight men are developing actual breast tissue, not just some extra padding. Doctors say that virtually all obese men (those who have a body mass index of 30 or greater, experience some degree of breast growth. And once that breast tissue is established, it doesn’t go away on its own—no matter how much you exercise or diet. Considering that 75 percent of Americans are likely to be overweight by 2015, according to a recent John Hopkins study, men may want to think twice about supersizing that cheeseburger.

Even if some older men have made peace with their manboobs, gynecomastia is no joke for many young men—particularly in a culture in which pecs of steel are the norm in the mainstream media. The problem with adolescent onset gynecomastia is that it’s happening at a time when kids are first becoming aware of their bodies. Kids at that point are really interested in fitting in, and this can make them targets for everything from emotional to sexual to physical abuse.

Impatient teens have few options. They can wait it out and hope the gynecomastia goes away as hormone levels fluctuate; most doctors say to give it two years or so. If it still persists, a doctor can prescribe an estrogen-blocker—a common one is the breast cancer drug Tamoxifen—to reverse the hormone levels. Or, as an increasingly common last resort, there’s breast-reduction surgery, a procedure some 20,000 American men—including 14,000 teenagers—underwent last year, making it the fifth-most common male surgery in America, according to the American Society of Plastic Surgeons.

Plastic surgeons say the procedure today is easier, safer and more effective than it’s ever been. The surgery ranges in price from about $4,000 to $10,000 and can take as little as an hour, depending on the size and makeup of the breast, which can range from puffed up nipples to more fully formed breasts. In some cases surgeons use liposuction to remove fatty deposits and glandular tissue, all through a “tiny nick in the skin”—about an eighth of an inch. In more severe cases a surgeon may need to open up the breast to remove tissue and excess skin, and occasionally even reposition the nipple.

Even if the condition doesn’t merit surgery, doctors are hoping that more men seek medical advice about treatment options instead of resigning themselves to a life lived with their shirts on. Talking about the problem, they say, is key. Raising awareness of the condition—its causes, symptoms, treatments—will help more boys and men with gynecomastia be correctly diagnosed, and reassure them they are not alone.

“Bob had bitch tits”, or so the movie character famously said. This opening one-liner from Fight Club is perhaps the most mainstream mention of gyno. It introduced a generations of moviegoers to the horror that is male breast tissue growth, although many were already familiar with it.

Gyno – What is it?

Gynecomastia is a benign enlargement of the breast tissue in males. It occurs naturally in about 1/3 of those who suffer from it, with the other 2/3 from drug use, usually anabolic steroids. Once it occurs, it is permanent, and can only be removed via surgery. It is caused by sensitivity to estrogen in the body, and is often spurned when stopping a steroid cycle.

Surgical Solutions

Gynecomastia is correctable with surgery and other various conditions. Liposuction, reduction mammoplasty, skin sculpture, and gland excision are all commonly used. Typically, a plastic surgeon will place a small incision at the border of the nipple complex. The glandular tissue is then removed. A vest is worn for several weeks to ensure healing occurs and the shape of the chest remains constant. Many bodybuilders – even the new young pros we see in magazines – opt for this procedure in the off-season during a break.

Over-The-Counter Remedies

The surgical solutions above are designed to remove gyno once it has become a major problem. Preventative medicine is always preferable to surgery. Gyno occurs during the weeks following a steroid cycle. Bodybuilders employ a technique known as PCT, or post-cycle therapy, in which two compounds are introduced to the body to correct the estrogen/testosterone imbalance. PCT consists of 1-2 AIs (Aromatase Inhibitors) and 1 SERM (Selective Estrogen Receptor Modulator) used to help correct the balance during that initial imbalance period, until the body begins producing testosterone on its own and the estrogen production slows. The top over-the-counter solution is probably Nolvadex-XT by Gaspari Nutrition. It is very popular with amateur bodybuilders and those without a doctor.

Gynecomastia, like acne or baldness, is a condition that comes with the territory when it comes to using steroids for competitive bodybuilding purposes. The trade-off can be minimized, however, if the user knows beforehand that gyno is indeed an occurring side effect, and takes preventative measures to ensure its impact is minimized. Too many bodybuilders run headfirst into the steroid realm without a true understanding of the side effects. Use of PCT, along with prompt surgery when that fails, ensures that one doesn’t end up like our good friend Bob of Fight Club fame.

Clomid is an interesting drug.

In bodybuilding circles, Clomid is used to increase lean muscle mass and reduce fat. In the medical world, Clomid is useful for bodybuilders, and the general population as well, when they need a little help starting a family.

For men suffering from fertility issues, particularly low sperm cell count, Clomid can be a godsend. Men who use anabolic steroids in their 20s and 30s often suffer from low sperm count. Their testes halt production of testosterone and sperm cells, due to the fact that artificial testosterone is detected in the body. These men will possess high T-levels, but they will not be able to reproduce as a result of the testicular shutdown. As many men reach their 30s, they are ready to start a family. Clomid helps kickstart sperm cell production in males.

Scientists still cannot agree exactly how Clomid works. It’s fairly commonly accepted that Clomid convinces the brain that estrogen levels are low. As a result, the brain produces more estrogen, and more testosterone. Blood levels of natural testosterone rise and sperm cell counts rise as well. Usually within three months, men regain their ability to produce enough sperm for impregnation.

Are there side effects? Of course! Clomid causes gynecomastia, or the very dreaded “bitch tits”, a swelling of the fat around the nipples, which looks terrible on the bodybuilding stage when men diet down.

Clomid is not a steroid. It has no real androgenic properties. It doesn’t alter testosterone production by the testes, nor does it affect the endocrine system in any way. Some researchers believe longer-term Clomid use causes tumors, but there isn’t enough data yet available to make such a determination. Many bodybuilders use it because it does affect T-levels, and it is easily accessible, but aside from that, its place in a cycle is usually unnecessary. It should be used for muscle gaining purposes when no other compounds are available.

The FDA has not yet approved Clomid for fertility assistance, but it use has been practices for many years now. Bodybuilders and researchers know what the FDA usually takes much longer to recognize, Clomid helps bodybuilders regain ability to reproduce. Use it if you’re trying to start a family and your doctor determines your sperm count is just too low. If you’re looking to gain muscle, use it as a last resort, but work to gain access to testosterone and other more effective compounds. Happy training and parenting!
